Is this cat primarily indoors or outdoors?
Is this cat friendly, easily handled and can be picked up?
Do you have a hard shell carrier to transport the cat in? if not, one may be loaned. Each cat must be in a separate carrier.
If the cat is a neighborhood stray, are you the caretaker/feeder?
Please list any vet care the cat has received in the last 30 days.
Has your cat had a rabies vaccine in the last year? If so please list the vet clinic and date.
Does your cat have any known medical issues?
Cats will need to be kept indoors after surgery. Are you able to keep the cat inside for 24 hours?
Do you receive SSI or Disability?
Are you over the age of 60?
Are you a Veteran?
Are you employed?
Are you a single parent/guardian?
How many people live in your household?
If the application is approved, you will be contacted about further steps and the date of the surgery. We will have limited spaces available for each date. You will be asked to pay a $25 copay and a link will be provided to pay with a card.
Transportation of the cats to the clinic will be provided, but you will have to meet at the drop off and pickup locations at the times provided. All cats must be returned to their owner/location of the application. By submitting this application, you are agreeing that Lucky's Cat House is only assisting with spay or neuter, vaccines and flea prevention provided for the cats. Any additional medical charges or services will be the responsibility of the owner/caretaker.
Services to be provided/covered:-Sterilization surgery (including anesthesia and pain managment)-Rabies vaccine-FVRCP vaccine-Revolution flea/tick prevention (topical application providing flea prevention for 30 days)
Are you willing/able to bring the cat to Lucky's Cat House, 1191 E Commerce St, Lewisburg, TN, on the day of surgery? The drop off time will be between 6:30-7am.
Are you willing/able to pick the cat up after surgery at Lucky's Cat House between 5:00-5:30pm?
For Lawrence County residents: if you cannot meet for dropoff and pickup at Lucky's, would you be able to meet a volunteer at a designated spot in Lawrenceburg at 5:30am and 6pm?
Submission of this application does not guarantee a spot. We plan to have multiple days through April. If you cannot make the first scheduled clinic day, you will be placed on our list to contact for the next date.
