Owner acknowledges and agrees to the following requirements for the care and procedure of the Animal(s):

Female Cats Only: Murphy's Safe Haven (MSH) will only perform spay/neuter services on female cats. Male cats will be denied service.
Weight Requirement: The Animal(s) must weigh at least 3 pounds. Any kittens or cats not meeting this weight requirement will be denied service and must be picked up immediately.
Health Requirements: The Animal(s) must be in good health. MSH will not perform surgery on cats that exhibit signs of illness or health concerns, including, but not limited to, sneezing, vomiting, diarrhea, mange, or any other visible health issues. The Animal(s) must be free from any condition that could affect the safety of the surgery.
Vaccination and Health Status: The Animal(s) must be up-to-date on required vaccinations or receive them as part of the service if necessary.
Murphy's Safe Haven reserves the right to refuse service at any time for reasons including, but not limited to, failure to meet these requirements, health concerns, or other factors that may affect the safety and well-being of the Animal(s) or staff. This decision is at the sole discretion of MSH, and no refunds will be provided in cases where service is refused.

This Agreement is made and entered into by and between Murphys Safe Haven and the undersigned, regarding the spay/neuter procedure and associated care for the cat(s) identified in this form above. By signing this Agreement, the Owner acknowledges, understands, and agrees to the terms and conditions set forth herein.

Outdoor Cat Status
Owner acknowledges and agrees that the Animal(s) is/are outdoor cat(s) and will be returned to an outdoor environment after recovery from surgery. Owner agrees to ensure that the Animal(s) will be adequately cared for and monitored after surgery.

Mandatory Eartip and Microchip
Owner acknowledges that, as part of the spay/neuter procedure, the Animal(s) will receive an eartip and a microchip for identification purposes. The Animal(s) will not be eligible for release without these procedures having been performed.

Microchip Ownership and Transfer of Animal
Owner understands and agrees that if the Animal(s) is/are found to have a registered microchip, MSH reserves the right to contact the registered owner and, at MSH's discretion, return the Animal(s) to the microchip owner. This decision is final and binding.

Post-Operative Care Responsibility
Owner acknowledges that they are fully responsible for the post-operative care of the Animal(s). Owner agrees to follow all post-operative care instructions provided by MSH, including monitoring for infection, preventing tearing of stitches, and ensuring a safe recovery environment for the Animal(s).

Acknowledgement of Surgical Risks
Owner acknowledges that all surgeries, including spay/neuter procedures, carry inherent risks. These risks include, but are not limited to, injury, death, infection, or adverse reactions to anesthesia. Owner understands that the Animal(s) will be placed under general anesthesia and voluntarily assumes full responsibility for any consequences that may arise from the surgery, including those arising from complications during or after the procedure.

Health Status of the Animal(s)
Owner affirms that, to the best of their knowledge, the Animal(s) is/are in good health and free from any previously diagnosed medical conditions that may affect the outcome of the surgery. Owner further affirms that the Animal(s) has/have not exhibited signs of illness, including vomiting, sneezing, coughing, or diarrhea, within the last two (2) weeks.

Fasting Requirement
Owner agrees to ensure that the Animal(s) is/are properly fasted for at least 7 hours prior to surgery. Owner understands that failure to comply with this requirement may increase the risk of complications related to anesthesia, including aspiration pneumonia. Owner acknowledges that MSH is not liable for any complications resulting from failure to observe the fasting requirement.

Pregnancy Acknowledgement
Owner understands that if the Animal(s) is/are found to be pregnant at the time of surgery, the pregnancy will be terminated without notification to the trapper. Owner agrees to the termination of pregnancy as part of the surgical procedure.

Pick-Up Requirement and Abandonment
Owner agrees to pick up the Animal(s) on the date and time specified by MSH personnel following the surgery. Failure to retrieve the Animal(s) within one (1) day of surgery will be deemed abandonment. If the Animal(s) is/are not picked up within this time frame, an overnight charge of $20 per night will apply. If the Animal(s) is/are not picked up within 72 hours after the scheduled pick-up time, MSH reserves the right to consider the Animal(s) surrendered and may place the Animal(s) up for adoption or take any other necessary actions.

Bite History
Owner affirms that, to the best of their knowledge, the Animal(s) has/have not bitten any person within the last fifteen (15) days prior to the date of this Agreement.

Recovery Environment and Post-Surgical Care
Owner agrees to provide the Animal(s) with a clean, dry, and safe environment for recovery. Owner agrees to adhere to all post-surgical care instructions provided by MSH and ensure that the Animal(s) is/are monitored appropriately during the recovery period.

Additional Charges for Extra Services
Owner understands that additional charges may apply if the attending veterinarian determines that the Animal(s) require any additional medical services, tests, or treatments beyond the standard procedure.

Resolution of Disputes and Non-Disparagement
In the event of any dispute between Owner and MSH, Owner agrees to work directly with MSH management to resolve the issue in a timely and mutually satisfactory manner. Owner further agrees not to disparage MSH in any public forum, including social media, online reviews, or other internet-based platforms. For the purposes of this provision, "disparage" shall mean any negative statements, whether written or verbal, concerning MSH or its services.

Entire Agreement
This Agreement constitutes the entire understanding between the parties and supersedes all prior discussions, representations, or agreements, whether written or oral, related to the subject matter hereof. No amendment or modification of this Agreement will be valid unless made in writing and signed by both parties.

Severability
If any provision of this Agreement is determined to be invalid, illegal, or unenforceable, the remaining provisions will remain in full force and effect.

By submitting this form, I understand and agree that if my request is approved, I will be contacted via phone or text message from (956) 778-0750 to schedule a spay appointment. Once my request is approved, the full cost of the surgery and any additional services must be paid in advance within 24 hours to secure the appointment. I acknowledge that this payment is non-refundable, and failure to submit payment within 24 hours will result in my request being denied."
