Are you opting to have your pet fixed by Grass Roots Rescue or will you be handling?
Name of pet your are fostering with the intent to adopt:
Do you understand that if the animal is not fixed in the allotted time frame, we reserve the right to come in and remove the animal?
Foster Intending to Adopt understands and agrees that GRR is the sole owner of any animal until it has been spayed/neutered and the final Adoption Contract has been completed. Do you agree?
Financial responsibility for the care of the animal (both every day costs, and medical with the exception of the spay/neuter) is the responsibility of the individual(s) Fostering to Adopt, unless otherwise agreed upon.
This includes, but is not limited to, monthly treatments such as heartworm preventative and flea and tick control medications. You may purchase preventative from GRR during this time if needed. Do you understand and agree?
Do you understand that, if at any point, we feel as though the animal is not being cared for properly, we reserve the right to come in and remove the animal?
Foster Intending to Adopt agrees and understands that the care, control, and custody of the animal may not, under any circumstances be transferred to any person without prior consent by GRR. Foster Intending to Adopt further agrees if for any reason they cannot continue to house and care for the animal, they will immediately notify GRR. The animal shall not under any circumstances be given away, sold, or adopted out to any third party.
We highly recommend the "2 week shutdown" when bringing a new dog home. Please confirm that you were informed of the 2 week shutdown which can be found here: https://k9ei.dog/two-week-shutdown-for-the-dogs-new-beginning/
Do you understand that ADOPTION FEES ARE NON-REFUNDABLE?
Do you understand that under current law the adoption is not finalized until the adoption fee is paid in full, dog is spayed/neutered, and the final Adoption Contract is completed? Until then, the dog remains the "property" of Grass Roots Rescue.
If the animal you’re Fostering to Adopt is not yet fixed, you must adhere to the following guidelines: Under 6 months of age: You must give us a $75 check deposit, which will be destroyed once the procedure is complete. The dog must be spayed/neutered by 6 months of age. An appointment must be scheduled by the time the dog is 4 months, and you must notify us of the appointment date. You are also responsible for sending us documentation confirming the animal has been fixed. Should this not be done as outlined here, your deposit check will be cashed and will not be refunded.
Dogs that are over 6 months of age: You must give us a $75 check deposit, which will be destroyed once the procedure is complete. A spay/neuter appointment must be scheduled within 14 days of taking the dog home. You must notify us of the appointment date, and are required to send us documentation confirming the animal has been fixed. Should this not be done as outlined here, your deposit check will be cashed and will not be refunded.
There is much debate about what the appropriate age is to have a dog fixed. We do not have a set stance on this, but we are required to follow the regulations put in place. Delaware code states that any animal ADOPTED from a shelter or rescue must be spayed or neutered by 6 months of age. Failure to do so can result in a $250 fine for the adopter and a $500 fine for the rescue. If you’d like to read more about the codes we operate under, you can do so here: http://delcode.delaware.gov/title16/c030f/sc02/index.shtml
If the animal you are adopting is not yet fixed, you must adhere to these guidelines. Do you agree that you have read and understand and agree to these conditions?
If already scheduled, you agree to keep the spay/neuter appointment and understand that the location cannot be changed. If adopting a puppy under 6 months of age, you agree to email us when your puppy is 5 months of age to schedule their appoint. You do understand and agree?
If your newly adopted pup already has a spay/neuter appointment scheduled, keeping that appointment is a condition of adoption that you have agreed to. If you miss that appointment, you will be in violation of your adoption contract, and will become responsible for the cost of the procedure once it is rescheduled. If you miss a scheduled appointment for a second time, you will be required to bring the dog back as they are still our “property” until the spay/neuter is complete.
If already scheduled, enter the date and location of the appointment below to acknowledge your understanding of the requirement and verify that you are aware of the date and location of the appointment.
If for a puppy under 6 months of age, type the month they will be 5 months old when you will need to email GRR for an appointment.
In the case of a return, if you take one of our animals anywhere other than to the designated location, you agree to pay Grass Roots Rescue a fine of $200.00.
Should you return a dog to us in poor condition and it is decided that the dog is in need of medical care, you agree to pay Grass Roots Rescue $300.00 towards the veterinary bill.
Foster intending to adopt agrees that GRR is not liable in any way for any injury or damage caused by the above referenced dog while in undersigned’s care, custody, and control. Foster intending to adopt fully and voluntarily agrees to release and indemnify GRR, its officers, directors, members, agents, volunteers, and representatives from any and all actions, claims, and liability resulting from or related to injuries or damage to persons, animals or property caused by or arising from any act or occurrence involving said dog while in the undersigned’s care, custody and control; including, without but not limited to, dog bites, illness, or any other action of said dog. This release of liability and indemnification provision includes, without limitation, injury or damage to persons and children who come in to contact with said dog while in the undersigned’s care, custody and control; and any personal pets and other animals. I HAVE READ AND UNDERSTAND THIS ENTIRE FOSTER TO ADOPT CONTRACT AND RELEASE OF LIABILITY AND AGREE TO ALL OF THE ABOVE TERMS AND CONDITIONS.
