We understand that surrendering a dog can be a difficult decision, and we appreciate you reaching out to us. In order to better understand your dog and assess if we can accomodate them, please complete this form in full. Please note that completing this form does not guarantee acceptance of your dog into our program. We will review your responses and contact you as soon as possible to discuss further steps. Most rescues, shelters, and breeders have "first rights" to their dogs if an owner decides to surrender the dog. Have you contacted the original rescue, shelter, or breeder?
What reasoning did the original rescue, shelter, or breeder have for not accepting their dog?
Has your dog received all necessary vaccinations?
Is your dog currently on any medication? Do they currently or have they ever had any medical complications or health issues?

Have you had any of the following experiences with your dog, either currently or in the past?

How does your dog react to new people or unfamiliar environments?
Has your dog ever been exposed to children? If so, how did they react? How old were the children?
Has your dog ever been exposed to a cat? If so, how did they react?
Is your dog house trained? Do they have any specific obedience commands or trainings that they respond to?
Is your dog comfortable being crated or confined? Where do they usually sleep? Where are they usually left when you leave the house?
Does your dog have any separation anxiety or destructive behaviors when left alone?
Has your dog ever tried to escape (jumping fences, opening doors, busting out of their crate, etc)? If so, how did you get your pet back?
How often does your dog nip/bite people or other animals? Please provide details of the circumstances, how long ago it happened, and any description of the injuries (ie., did it break skin, bruising, puncture, needed medical attention, etc).
Is your dog comfortable being handled, groomed, or examined?
How would you describe your dog's behavior at the vet? Does the vet need to muzzle or sedate your dog in order to handle them?
What is your pet's response to strangers (in the home and outside)?
How would you describe your pet's behavior when a person or animal comes near or takes away their food, treats, toys, or bones? (ie., do they freeze, lift their lip and show their teeth, growl, bite, etc?) At what distance is your pet triggered? When and how often does this happen? Please describe any injuries as a result of this behavior, and what you have tried to help address it.
What type of exercise and mental stimulation does your dog currently receive?
How is your dog in a car?
What are some of your dog's favorite things?
What are some of your dog's least favorite things?
What is the main reason you are surrendering this dog? What have you tried, if anything, to overcome barriers to keep them?
If your dog has any medical or behavioral concerns, and it's something that we could help treat, would you be interested and able to keep your dog instead of surrendering?
