In order to better understand your dog and assess if we can accomodate them, please complete this form in full. Please note that completing this form does not guarantee acceptance of your dog into our program. We will review your responses and contact you as soon as possible to discuss further steps. Thank you for considering Thrive as an option for your dog. Thrive typically charges a $350 Relinquish Fee per dog when accepted.
