HRN Request Surrender Application

House Rabbit Network will accept rabbits being surrendered if we have space for them. At the time of surrender you will need to sign our Surrender Agreement, which follows for your review:

House Rabbit Network Surrender Agreement

This document signifies that I am the rightful owner of the rabbits listed.

To my knowledge, no other person has any rights to this rabbit. I hereby relinquish this rabbit to the House Rabbit Network ("HRN") and relinquish all rights of ownership. I shall have no further rights to the rabbit. I understand that once I relinquish the rabbit, the rabbit will not be available to be returned, nor will I be able to determine its final placement. I understand that if I seek to reclaim this rabbit, I will have to go through the normal HRN adoption procedure.

HRN makes no guarantee that the rabbit will be successfully fostered or adopted. If the rabbit is terminally ill and suffering, HRN may, at its sole discretion, choose to euthanize the the rabbit.

Owner gives veterinary offices permission to release any and all medical records for the listed rabbits to the House Rabbit Network.

In an effort to make this parting, and the subsequent search for a new home, less traumatic, HRN requires surrendering owners to complete the HRN Request Surrender Application. To the extent possible, please also provide HRN with your pet's veterinary records, if any.

I have read and understand this Surrender Agreement and accurately provided HRN with the rabbit information to the best of my knowledge and ability.
