Thank you for your interest in fostering for Wonder Paws Rescue. This is a questionnaire to help you communicate with us what you are interested in doing, and also the Foster Agreement so you know what our expectations are, and what you can expect from us.Please remember that Wonder Paws rescues animals from many different situations. Some are coming out of a shelter environment, some have been mistreated and abused, and others may have never lived in a home before. All animals will need some time of adjustment into a new foster home, but your honesty on this form will help us match you up with a pet that will suit your home and potential limitations and/or strengths.

Unless otherwise discussed, Wonder Paws asks that our foster parents keep their foster animals until we find them a forever home (except in cases of emergency). We understand that some fosters take a little longer to get adopted. For example, a puppy/kitten (Short -Term) gets adopted rather quickly compared to senior dog/cat (Long-Term). Would you consider yourself a Short-Term foster or a Long-Term foster?
If the foster does not think the animal is a good match for their home, they agree to keep the animal in their care while Wonder Paws finds new placement for the animal.

As an Animal Foster Volunteer, you will be responsible for providing adequate food and fresh water, socialization, attention, affection, and a close eye on the animal's health.
The rescue supplies food and other supplies out of its rescue office in Fort Lauderdale. In order to receive food, you fill out a form requesting what you need and either pick it up yourself or find someone to pick it up for you. If your foster pet requires special food that you need to buy or getting food from the office will create a hardship for you, you must seek approval before buying if you wish to receive reimbursement, and then submit receipts once purchased to receive a reimbursement check in the mail.

If you wish to foster a dog and have a resident dog, please confirm that your pet dog is over 16 weeks and up to date on vaccines. Please note: if your resident dog(s) are not up to date on vaccines and not on a monthly heartworm preventative, you will not be approved to adopt your foster.
If you wish to foster a dog and have a resident dog, please confirm that your pet is spayed/neutered.
If you wish to foster a cat and have a resident cat, please confirm that your pet cat is over 16 weeks and up to date on vaccines

Are the pets in your home spayed/neutered?

Wonder Paws always does home visits for their foster parents to ensure it is a safe environment for the pet. This will be done before you get your foster pet, or at the time of receiving your pet. Do you agree to this home visit?

Wonder Paws will be allowed to reasonably inspect the premises in which the animal(s) will be maintained or are maintained, from time to time, for the purpose of determining the suitability of those premises for the care and maintenance of the animal(s). Please initial.
If you decide to become a "foster failure" and adopt your foster animal, in the best interests of the animal and the rescue, you will be required to follow the standard adoption approval process and pay the standard adoption fee. Do you agree?
You agree to surrender the foster animal(s) to WP when a permanent adoptive home is found. While foster parents' input is valued and solicited regarding prospective homes, Wonder Paws will ultimately approve or disapprove all prospective adopters. If a friend or acquaintance is interested in adopting your foster animal, they are required to follow the standard adoption approval process (application, home visit, etc) and pay the standard adoption fee.
Unless otherwise discussed, Wonder Paws asks that our foster parents keep their foster animals until we find them a forever home (except in cases of emergency). Depending on the adoptability of the animal, we expect this to take 2-4 weeks. If the foster does not think the animal is a good match for their home, they agree to keep the animal in their care while Wonder Paws finds new placement for the animal. As an Animal Foster Volunteer, I agree to provide proper and sufficient food and water, shelter, socialization and kind treatment. I agree to feed proper and sufficient food and water, bathe/groom, exercise, and socialize the foster animal(s) as appropriate. I agree to never hit, kick or abuse the animal in any way. I understand that I shall not leave the animal(s) unsupervised outside, and shall comply with all city, state and federal laws relating to animals. I also understand that I shall provide the above-described services without monetary compensation from Wonder Paws Rescue.
As an Animal Foster Volunteer, I agree to administer a monthly heartworm pill (dogs only) and a monthly topical flea medication provided by the rescue.
As an Animal Foster Volunteer, I agree to provide proper and sufficient food and water, shelter, socialization and kind treatment. I agree to feed proper and sufficient food and water, bathe/groom, exercise, and socialize the foster animal(s) as appropriate. I agree to never hit, kick or abuse the animal in any way. I understand that I shall not leave the animal(s) unsupervised outside, and shall comply with all city, state and federal laws relating to animals. I also understand that while Wonder Paws provides food and supplies at our office location, it may be possible for foster to purchase if there is a shortage at any time.
Foster agrees and understands that if any animal in their care requires emergency vet visits, due to their negligence, the rescue will hold the foster caretaker responsible for all fees incurred if it is determined by the Board the negligence was caused by the foster caretaker. Do you agree?
You consent to the rescue screening your adoption application, including some or all of the following: checking listed references; HOA/landlord/complex; and any searchable public record.
I agree to contact Wonder Paws immediately, and provide transportation, should veterinary medical care be needed for the animal(s). Wonder Paws works with veterinarians that are familiar with shelter/rescue medicine. If the foster animal needs medical care of any kind, I have the option to contact Wonder Paws immediately and to follow their instructions. Foster animals may go to a foster parent’s personal vet at the cost of the foster parent. Wonder Paws cannot be responsible for vet costs at unapproved veterinarians.
Our rescue veterinarians can be found on the volunteer website which will be provided to you upon approval of your application. We have veterinarians from Miami to St. Lucie County.

I agree to hold harmless Wonder Paws, its Board of Directors, their successors, heirs or assigns, any agent carrying out duties on behalf of Wonder Paws, and any and all participating persons who might be claimed to be liable, whether or not herein named, from any and all actions, causes of action, lawsuits, claims and demands which I now have or may hereafter have, arising out of, or in any way relating to, any and all injuries and damages, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to my participation as a Wonder Paws volunteer or foster home.
I fully understand that Wonder Paws makes no guarantees whatsoever as to the health, temperament, mental disposition and training of the animal(s) and understand that Wonder Paws makes no representations or guarantees about any animal’s temperament and that any comment that an animal may be good with children or other animals or is housebroken is based upon information provided by previous owners or shelter notes and not a guarantee. I am aware that dogs may cause property damage or personal injury to myself, my family or others.
