MAMCO Cat Adoption Contract

By initialing you indicate that you agree to and understand the following:
I understand that the MAMCO Rescue adoption fees include the following: Spay or Neuter; microchip; age appropriate vaccinations ; deworming; and continued support from our staff. I understand that the adoption fee also offsets the costs of transporting, feeding, vaccinating, spaying/neutering, and providing medical care for each cat/kitten. I understand that there may be a period of adjustment. I understand that I can call my adoption representative to receive advice and guidance following the adoption. I agree that if at any time my situation changes and I can no longer keep my rescue cat/kitten that I will return the cat/kitten to MAMCO Rescue, but a refund will not be issued. In agreeing to exclusively surrender the cat/kitten to MAMCO Rescue, I promise not to attempt to rehome my cat using social networking sites, Craigslist, or any other pet advertisement service. This includes rehoming to friends and family. I understand that MAMCO Rescue does not refund money under any circumstances. I understand that these cat/kittens originated in high kill shelters where contagious disease is prevalent and that while MAMCO has taken every necessary precaution to ensure the health of the cat/kittens, a large portion of their medical history remains unknown. I understand listed breeds are an estimation based on physical characteristics, but are not guaranteed. I understand that I am 100% responsible for all veterinary costs from the date of adoption, with exception of the FIV/FeLV testing which is covered by MAMCO at one of their partner veterinarians, 30 days post adoption. I understand that MAMCO Rescue has made every effort to ensure my companion’s health before adoption with full vetting and age appropriate vaccinations. MAMCO is unable to predict genetic defects that may impact a cat/kitten. It is my responsibility as the adopter to begin my cat/kittens on preventatives and other necessary wellness treatments to keep the cat/kitten healthy for the long term. I understand that MAMCO is not responsible for the veterinary costs of my adopted animal(s), but that I can contact the rescue via email for help and guidance at any time. I understand that MAMCO provides me with detailed instructions on how to care for my puppy and that a failure to follow those instructions may be detrimental to the health and well-being of my adopted animal(s). I understand that MAMCO does not have the ability to predict every situation that will arise regarding the health, genetics, personality, behavior, and/or appearance of the rescued cat/kittens. I understand that actions taken on my part to slander or libel MAMCO publicly or privately due to factors about the cat/kittens which could not reasonably be predicted prior to adoption will result in legal action. I agree to make every attempt to work with MAMCO to resolve health and/or behavioral issues in lieu of publicizing negative reviews or comments. I understand that MAMCO takes its relationships with adopters very seriously. I understand that MAMCO will work towards a peaceful resolution in accordance with all the terms outlined in this contract. I understand that MAMCO Rescue requires adopters to be 21 years of age or older, and by signing this contract I confirm that I am 21 years of age or older. By initialling this box you agree that you have read and understand the terms above.

As a responsible adopter, I agree to do the following: have my companion vaccinated annually and treated immediately for any illness/injuries; provide my companion with clean water, healthy food, shelter in a loving, humane home, and will not leave him/her unattended for extended periods. I understand and agree to house my companion indoors at all times, unless they are being walked on a leash or being transported in a carrier, and not allow him/her to run unsupervised. I agree to never leave my cat/kitten alone in a vehicle when it is above 70 degrees or below 40 degrees outdoors. I promise to never physically harm the cat/kitten. I agree to take the animal(s) to a veterinarian to receive routine medical care and when the animal(s) is ill. I agree not to abandon the animal(s) with any party, or to rehome without first contacting MAMCO. I agree that any failure in the aforementioned items constitutes a breach of contract. In the event of any breach, I agree to relinquish the adopted animal(s) immediately to MAMCO Rescue, I understand that no monies will be returned to me. I have received my pet’s vaccination/medical history and understand that additional vetting may be necessary, especially for kittens.

I have received a copy of the Colorado Department of Agriculture’s rabies prevention brochure.

I agree to give my newly adopted animal a minimum of two weeks to adjust to our home.

I understand that if I choose to return my cat/kitten without giving the cat/kitten a minimum adjustment period of two weeks, that I will not be allowed to adopt from MAMCO again in the future.

I understand that regardless of the circumstances surrounding the surrender, I will not receive a refund of any portion of the adoption fee if I choose to bring back the cat/kitten.

I understand that should I re-home the cat/kitten illegally through any other avenue besides MAMCO, that I will be liable for all attorney's fees, court costs, and reclaiming costs associated with reclaiming the cat/kitten.

I understand that if I choose to bring my cat/kitten back and it has been more than 5 months since the adoption, that I will be required to pay a $100 surrender fee. It costs the rescue money to have cat/kittens returned back into our care. Please initial that you agree to pay a $100 surrender fee should I choose to return my cat after owning the cat/kitten for 5 months or more.

I understand that if I decide to surrender my pet, I must agree to give MAMCO a minimum of 4 to 6 weeks to find a foster home for the animal. We are foster based and cannot place returns the same day. In the event that I cannot wait the required 4 to 6 weeks, I agree to pay boarding in the amount of $50 per day for up to 4 weeks.

I understand that it is my responsibility to activate the 30 day pet insurance within 24 hours of adoption or by Monday following a Saturday adoption, and that the coverage is not retroactive.

1.I understand that if at any time in the future I decide that I can no longer keep my cat/kitten, that I am required to surrender the cat/kitten back into the care of MAMCO Rescue.

2.I understand that I will NOT receive a refund of adoption fees if I choose to surrender the cat/kitten at any time following the adoption.

3. I understand that my eligibility to adopt from MAMCO in the future following a surrender, is up the discretion of the MAMCO board of directors.

4. I understand that the MAMCO board of directors will take the circumstances surrounding the surrender into consideration before making a final decision, but that the following reasons for surrender will result in an immediate denial of future adoption applications:

5.I understand that if the MAMCO board allows me to adopt again, no credit will be awarded for future adoption fees.

6. I understand that it is a breach of contract to rehome the cat/kitten on my own using social networking sites, Craigslist, or any other pet advertisement service. This includes rehoming to friends and family.

7. I understand the each of the microchips is automatically tied to MAMCO rescue and any attempts to rehome the cat/kitten

without MAMCO knowledge will be reported to the rescue. Please see the adoption contract for specifics details regarding abandonment.

8. I understand that I am entering this agreement willingly and I agree to refrain from making disparaging comments about MAMCO's policy either publicly or privately. I understand that I will be financially responsible for any legal action against me if I should slander or libel the MAMCO refund and return policy.

I understand that in the event I abandon my animal(s) I will be responsible for all fees associated with MAMCO Rescue’s recover of the animal(s) including but not limited to transportation, necessary medical care, shelter fees and boarding. If litigation is required to recover these costs, I will be responsible for reasonable attorney fees and all costs of litigation.

I agree that in the event that I must surrender the cat/kitten back to MAMCO that I will assume all charges stated in the contractual obligations upon surrendering the cat.

I agree to assume full financial responsibility of the adopted animal(s) upon signing the contract.

I agree to hold MAMCO Rescue free from any and all liabilities arising from this animal. I agree to assume all responsibility for the defenses of any action that may arise as a result of my ownership.

I agree to pay any and all costs necessary to enforce this contract, including, but not limited to reasonable attorney fees and all cost of litigation.

I would like to make an additional donation of _________________to MAMCO Rescue in order to help MAMCO rescue more pregnant and nursing dogs.

I understand that MAMCO Rescue has the right to reclaim the cat/kitten if payment is returned due to insufficient funds or fails to be authorized by the bank.

I understand that my application was accepted because I indicated that I will care for, and love this cat/kitten for the remainder of its natural life. I understand that my application would not have been approved if I had not indicated that I would give the cat/kitten as much time as needed to adjust to my home and family. I understand that if I choose to relinquish the cat/kitten back to MAMCO due to reasons that are found unacceptable by the board of directors, that I will not be permitted to adopt from MAMCO again in the future.

I understand that adopting a cat/kitten is a serious commitment, and that MAMCO Rescue does not view these cat/kitten as property to be bought and sold. Rescue cats are valuable members of the family who should be cared for and cherished from this day forward. We understand that there are times when a cat/kitten may need to be returned, but the physical or genetic characteristics of the cat/kitten, and not giving the cat/kitten enough time to adjust are not reasons that we encourage.

I certify that all of the information provided in my adoption application is true and factual. I understand that misrepresentation of myself in the application process could result in a breach of contract.

1. Incoming cat/kittens are quarantined in foster homes for a minimum of 10 days before adoption. Most contagious illnesses have a 7-10 day incubation period. This quarantine period may have taken place in a foster home in another state.

2. All incoming cat/kittens are vaccinated and dewormed.

3. Vaccination records are obtained for the personal pets of foster parents to ensure the safety of the foster cat/kittens.

3. We work closely with our veterinarian to ensure that unvaccinated or under vaccinated cat/kittens are not exposed to illness. Therefore, many of the medications administered to our cat/kittens are administered in house.

9. All cat/kittens transported in from out of state receive a health certificate by a licensed veterinarian prior to entering Colorado.

10. I understand that I am adopting a pet that originated in a high kill shelter where tick born and viral illnesses are prevalent, and that MAMCO has observed safety protocol to help ensure the health of the pet.

By signing this section of the contract you agree that MAMCO rescue has taken reasonable precautions to ensure the health and well-being of the cat/kittens prior to adoption.

I certify that all of the information supplied in the application is true. I understand that my application was approved based upon the information that I provided and if it were discovered that any false information was provided, it would constitute a breach of contract.

By signing this section you agree not to unnecessarily alter your pet by docking ears, docking tails, declawing, or any other cosmetic procedure. Dewclaw removal is accepted in the event that it causes issues to the cat/kittens long term health and wellbeing. Please Sign Below:
