Surrender Form

For dogs adopted through OAAR:

The cost to care for and to rehome pets can cost hundreds to thousands of dollars for each dog. Rehoming a dog is taken very seriously, and owners are expected to exhaust all available options, including working with a reputable trainer, prior to surrendering an animal. In the unfortunate event that it becomes necessary to surrender your pet, the owner is responsible for a surrender fee to help subsidize our costs. A non-refundable $250 is applicable to all dogs accepted into the rescue for rehoming purposes.  The surrender fee should be remitted via Venmo (@ollliesangels) or Paypal (Info@OlliesAngels.org). After submitting this form, you must email a minimum of 5 clear photos of the dog within 24 hours to:  Info@OlliesAngels.org with the subject: Surrender +Dog's name + Your Name.  All medical records must also be mailed to Info@OlliesAngels.org. If we do not receive pictures, medical records, and the rehoming fee, we cannot start the rehoming process. *As a foster based rescue, Ollie's Angels Animal Rescue retains the right to refuse to rehome a dog deemed to be dangerous or aggressive towards humans or other animals.*

For dogs not adopted through OAAR:

If your dog was not adopted through Ollie’s Angels Animal Rescue, filling out this form does not guarantee that OAAR has the space or ability to accept the dog into the rescue. Our ability to re-home dogs not adopted through Ollie’s Angels Animal Rescue is limited. For all dogs: 

If deemed a candidate for rehome, all dogs must be up to date with: DHPP vaccine, Rabies vaccine, flea/tick preventative, heartworm preventative, a heartworm test performed with the past 6 months, and a general health exam performed within the past 12 months. If not up to date in any of these areas, it is the owner's responsibility to update the dog at their expense and provide documentation to OAAR.

To be accepted as a rehome, all dogs must undergo an evaluation by an OAAR representative or an OAAR approved trainer within two weeks of submitting the intake form.

Terms of Agreement

Release and Transfer: If the dog is accepted into the rescue, The Owner hereby relinquishes all present and future rights and claims to the Animal. The Owner hereby transfers the ownership of said Animal to Ollie’s Angels Animal Rescue for the purpose of having Ollie’s Angels Animal Rescue care for the Animal or search for a new home for the Animal. The Owner understands and acknowledges that upon execution of this Agreement, the Owner does not have the right to make any future decisions concerning the Animal, including, but not limited to medical and adoption decisions. The Owner hereby releases Ollie’s Angels Animal Rescue from any and all liability arising from the placement for adoption or any other necessary or appropriate action undertaken by Ollie’s Angels Animal Rescue on behalf of the Animal. Indemnification: The Owner agrees to indemnify and hold Ollie’s Angels Animal Rescue, its directors, officers, employees, representatives, volunteers, agents, subsidiaries, parents, affiliates, agents, and representatives from any and all liability that may result from the following: (a) the Owner not having the legal right to transfer ownership of the Animal; (b) claims made against the owner of the Animal that occurred prior to the execution of this Agreement; and (c) any misrepresentation about the Animal, including, but not limited to the Animal’s behavior or medical history that was provided in the Ollie’s Angels Animal Rescue Surrender Form. Rehoming: The Owner agrees that if Ollie’s Angels Animal Rescue agrees to take possession of the Animal, removing the Animal from his/her current home is contingent upon securing an appropriate foster or forever home. OAAR is unable to guarantee a specific time frame, as this is dependent upon the availability of a suitable foster or forever home. I agree to retain custody of the Animal until an appropriate foster or forever home is found.

Post-Surrender Confidentiality. After surrendering the Animal, the Owner shall hold in confidence and keep confidential all Confidential Information and shall not directly or indirectly, in any manner or for any purpose whatsoever, use, copy, recreate, reproduce or allow to be used, copied, or reproduced for the benefit of, or disclose, transmit, transfer or communicate or allow to be disclosed, transmitted, transferred or communicated to, any person any Confidential Information. For greater certainty, this prohibition shall extend to prohibit the Owner from further discussing the Animal or disclosing Confidential Information on social media or otherwise.

Digital Signature: Each party agrees that this Agreement and any other documents to be delivered in connection herewith may be digitally signed, and that any digital signatures appearing on this Agreement or such other documents are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.

Terms of Release: My signature below reflects that I have read and understand the information provided above, and I am hereby releasing all rights for this animal to Ollie's Angels Animal Rescue.
