Pet owner must be a resident of Dawson County, Montana
The owner of the pet must apply for the voucher.
The applicant must be 18 years of age or older.
Vouchers may not be used for pets adopted from Happy Tails, Inc.
The information I have provided is true and correct. I also understand that any false information provided will result in permanent termination from this program.
I agree to all eligibility requirements listed above and certify that I am the owner of the pet listed on the application.
The veterinarian performing the surgery has the right to deny or refuse to perform surgery on the pet for any reason.
I agree to indemnify, hold harmless and release all liability of Happy Tails Inc and any participating volunteers, and any of their personal representatives, heirs, successors and assigns, from and against all actions, claims, damages, disabilities, or expenses, including attorney's fees and witness costs that may be asserted by any person or entity, including me, arising out of or in connection with the care, treatment, surgery, or safe keeping of the pet(s). Further, I understand that it is not possible for you to guarantee that any medical or surgical procedure will be successful and without complication, including, but not limited to, the death of my pet(s). I understand these risks and assume all responsibility for such complications and will not hold Happy Tails Inc and any participating employee or volunteer responsible.

Vouchers are limited to one per household, per year.
Vouchers cannot be used for pets that are adopted from Happy Tails, Inc.
Vouchers will expire 3 months from the date of issue.
The applicant will pay the amount on the voucher to the veterinary clinic and Happy Tails will pay the remainder of the bill for the spay/neuter surgery only. Vaccinations and any other additional costs are the responsibility of the pet owner.
