Waco Pets Alive! Volunteer and Foster Application and Release

Waco Pets Alive! ("WPA!") is an all-volunteer nonprofit animal welfare organization located in Waco, TX whose mission is to provide the resources, programs and education needed to eliminate the killing of at-risk animals at the Waco Animal Shelter. WPA! is a small, but mighty, supportive organization who is always seeking new volunteers and fosters!

I understand and agree that as a volunteer or foster for Waco Pets Alive! (herein after referred to as "WPA!"), I assume all risks of loss or injury, including death to myself or damage to my property while participating with WPA! activities and elsewhere, while participating in the volunteer program.

I understand and agree that all services performed by me will be performed on a strictly volunteer basis, and that I will receive no remuneration, pay or compensation of any kind.

I understand and agree that I will not be an employee of WPA! and will not receive any benefits normally available to employees of WPA! I understand and agree that WPA! shall incur no liability of any nature as result of my volunteering for WPA!

I understand that in handling animals and performing other volunteer tasks there is a risk of injury, including physical harm or death, and that all services performed by me will be done at my own risk. I understand that WPA! strongly recommends that I keep current my tetanus immunization. I further understand that WPA! recommends that any dogs and cats that I live with should be immunized by my veterinarian, if not already done so.

Therefore, on behalf of myself, my heirs and personal representatives, I hereby release, discharge and indemnify and hold harmless WPA! and its assigns, successors, agents, staff, officers, board of directors, employees, contractors and representatives from any and all claims, causes of action or demands of any nature of cause whatsoever, including costs and legal fees arising out of, or relating to, my volunteering with WPA!, including but not limited to, animal bites, disease, accidents, property damage, or injuries.

1. I will abide by the mission, rules, regulations, policies and programs of WPA! while I am a volunteer.

2. I will treat all animals, other volunteers, and the general public with dignity and respect.

3. If I will be sheltering or providing foster care or boarding for any of the WPA! animals in my home, I understand that WPA! remains the legal owner of these animals and may remove them from my care at any time.

4. I have accurately and truthfully completed this Volunteer Application and Agreement.

5. I understand that WPA! volunteers must be at least 13 years of age. Volunteers age 13-15 must be accompanied by a parent or guardian at all times. 16 and 17 year olds may obtain a signed parent waiver in order to volunteer by themselves.
The parent or guardian must also complete this online application and attend volunteer orientation in all cases.

If age 13 - 17, enter the name of the parent or guardian who will be volunteering with you.

NOTE: Your parent or guardian must also fill out a volunteer application and attend orientation.

The parties acknowledge and agree that this Adoption Contract may be executed by electronic signature, which shall be considered as an original signature for all purposes and shall have the same force and effect as an original signature. Without limitation, “electronic signature” shall include faxed versions of an original signature or electronically scanned and transmitted versions (e.g., via pdf) of an original signature.
