Furry friends relinquishment form

I certify that I am the legal owner of the pet described in this document and I have the full legal authority to surrender this pet to Furry Friends Cat Shelter. By acknowledging below I certify that my intent is to surrender my pet and authorize Furry Friends to contact my Vet to obtain any previous vet records. I understand that my cat will be put up for adoption from Furry Friends.

I would like to donate an additional amount to help with my cats care until adoption or to help other cats in Furry Friends care.

I certify that I have given all known health information about my cat to furry friends. I understand if Furry friends finds out I withheld important health/medical information regarding my cat I can be held liable for vet bills in relation that the information.

I understand there is a $75 relinquishment fee that will go toward the care of my cat until adoption and that it needs paid at the time of relinquishment.
