Volunteer Waiver and Non-Disclosure Agreement

I hereby agree to be legally and financially responsible and will indemnify and hold MatchDog Rescue harmless for my own acts and omissions relating to the service I am voluntarily providing to MatchDog Rescue. I acknowledge that I am responsible for providing my own medical, liability, and auto insurance during my volunteer service. I understand that I am not covered by workers’ compensation nor am I insured by MatchDog Rescue during the performance of my volunteer duties and tasks.

I am knowingly and voluntarily participating in the activities of MatchDog Rescue with full knowledge and appreciation of the risks and dangers involved with animal care and hereby agree to accept all risks of injury, death or damage to myself and/or my personal property associated with my volunteer work. As a Volunteer, I will come into contact with and interact with dogs, and such work has an inherent risk of personal injury. Risks of injury associated with the position include, but are not limited to, being bitten, falling or subject to trauma, and possibly exposed to animal borne diseases.

I understand that as a Volunteer of MatchDog Rescue, I may be recorded on film, video or other electronic recording media. I hereby consent to such recording and the use by MatchDog Rescue of any recorded images or other media recordings of my name and likeness (“A-V Recordings”) for any purpose related to furtherance of the objectives of MatchDog Rescue. I further grant MatchDog Rescue permission to copyright and use, reuse, publish and republish A-V Recordings, for art, advertising, or other purpose it sees fit.

In exchange for being permitted by MatchDog Rescue to participate in activities and provide services, I hereby agree that I, my assignees, heirs, guardians, and legal representatives will not make a claim against or sue MatchDog Rescue for injury or damage resulting from my participation in activities and performance of services.

During my association with MatchDog Rescue and thereafter, I agree to keep confidential and refrain from disclosing to others all confidential or proprietary information of MatchDog Rescue that I create, learn about or utilize during the course of my association. I understand that this agreement covers all confidential business and technical information of MatchDog Rescue which is not generally known to persons outside of MatchDog Rescue and which I have not been specifically authorized to disclose or use. Examples of confidential information include, but are not limited to, information on finances, donors, outreach programs, internal meetings, discussions, private emails, text messages, and social media threads, as well as information contained in MatchDog Rescue’s data systems of record.

I agree not to wear attire or advertise my association with MatchDog Rescue with regard to my participation in any political statement, protest or organization. I also agree not to modify, distort or change the MatchDog Rescue logo on any clothing or attire I wear in public.

I have not been convicted of a violent offense relating to domestic violence, sexual violence, child endangerment, assault, animal cruelty, or armed robbery.

I am currently under no court order restricting my participation in any youth or animal welfare organizations.
