We, (INSERT ORGANIZATION"S NAME) , hereby voluntarily and irrevocably transfer to the animals listed below to Desert Cat Rescue & Sanctuary of Arizona, hereinafter referred to as DCRSA, the following animal(s), hereinafter referred to collectively as “Animal(s)”.

We represent and warrant that we are the lawful owner of the Animal(s), and we have full power and authority to transfer the Animal(s) to DCRSA. No other person has any legal or equitable ownership interest in the Animal(s).

We further acknowledge that we are transferring the Animal(s) completely voluntarily and that no representations, considerations or promises of any kind have been made to me by DCRSA or any of its representatives.
We understand that by transferring the Animal(s) to DCRSA, We relinquish all ownership or other interest in the Animal(s). DCRSA, if requested in writing, may provide general wellbeing updates on all transferred animal(s) within reason. We acknowledge upon adoption of transferred animal(s) DCRSA will not provide any wellbeing updates for animal(s).

We have disclosed to DCRSA all material information regarding the medical and behavioral history of the Animal(s). We willfully transfer all medical records and information pertaining to the Animal(s). DCRSA has our rescue’s permission to contact the veterinarian for all pets surrendered for any necessary information pertaining to the Animal(s), and we hereby consent to the release of any and all medical information by any medical provider. We also agree to provide any and all medical documentation that is readily available upon transfer of animal(s).

We hereby release and forever discharge DCRSA from any and all rights, claims, obligations, liabilities, and causes of action whatsoever arising out of or relating to the ownership, possession, or disposition of the Animal(s), and We agree to indemnify and hold harmless DCRSA from and against any and all such rights, claims, obligations, liabilities, and causes of action which may be asserted by third parties.

We hereby certify that the animal(s) has not bitten or scratched a human or another animal within the past 10 days.

We have fully read and understood that transfer of animal(s) is permanent upon completion of transfer. We agreed to all terms listed in the above rescue to rescue transfer form.
