This is an agreement for the adoption of:

between (hereafter referred to as "Adopter") and Waggytail Rescue:

The Adopter understands that the adoption fee is a donation and is not a sale price, and is non-refundable under any circumstances including if adopter returns the Animal. This contribution allows Waggytail Rescue to continue to save the lives of homeless, neglected, and sick animals.

Adopter will provide the animal with access to food, fresh water, affection, exercise, training, grooming, indoor shelter, and agrees to treat them like a treasured member of the adopter's family.

Adopter confirms they are adopting the animal for companionship and not for any other individual or organization and that this animal will not be used for fighting, protection, or for any form of profit.

Adopter agrees not to abuse or physically harm this animal in any way. Adopter agrees not to euthanize the animal except in the case of terminal illness, injury, or old age accompanied by pain and suffering. Euthanasia must be performed by a licensed veterinarian.

Adopter agrees to be financially responsible for this animal and provide proper veterinary care, including regular wellness visits and vaccinations, as well as any necessary emergency care due to illness or injury.

Adopter understands that with domesticated animals such as dogs and cats, that modifying certain behaviors may require training, expert advice, and patience. Adopter agrees to work with the Animal to modify any such undesirable behavior including, but not limited to: barking, aggression, house soiling, biting, scratching (people, furniture, woodwork), digging, mounting, urine marking, etc. - Adopter also understands that certain behaviors may never be entirely eradicated.

Adopter understands that pet adoption is a commitment for the lifetime of this animal which could be 12 years or more. Adopter agrees to contact Waggytail Rescue in the event their family or designee can no longer care for the animal. Adopter agrees that the animal will never be given to an animal shelter, research facility, or abandoned.

Adopter agrees that if any of the terms of this agreement are not satisfactorily met, Waggytail Rescue may reclaim this animal. Adopter understands that in the event they wish to return the animal, they must make prior arrangements with the rescue regarding date, time, and place and that any travel or boarding arrangements shall be at Adopter's expense.

Except as set forth above, Waggytail Rescue makes no representations, warranties or guarantees and assumes no liability for the health, age, sex, temperament, behavior, or condition of the animals available for adoption. Waggytail Rescue is operated for the benefit of the animals that we place. If any problems or questions arise as to the behavior or care of your animal, we welcome and encourage your inquiries.

Waggytail Rescue has communicated everything known about this animal's health and behavior, based on medical records, vet visits, and notes from the foster, to the adopter. Adopter has acknowledged any pre-existing conditions and is willing to adopt the animal "as is." The Adopter assumes all responsibility for treatment of any and all existing and future medical conditions and/or any other physical, behavioral, or temperamental issues of the animal now and in the future.

Adopter hereby waives any claim against, and releases and agrees to indemnify, defend and hold harmless Waggytail Rescue, its employees, agents, volunteers and fosters, from and against any and all liability arising from claims, demands, or causes of action, of any nature or kind, arising out of the adoption or adopter's actions or inactions with respect to this animal, accepting for themselves the full responsibility of any and all such losses, damages, injuries, and expenses.

If animal is unaltered, Adopter agrees to pay an additional $100 refundable spay/neuter deposit. The Animal must be altered within 3 months from adoption day. Under special circumstances, exceptions can be made through communication with a Waggytail representative.

Adopter agrees to pay the following adoption donation/fee:

If animal is scheduled for a neuter appointment communicated in the approval email, the Adopter agrees to bring the animal to and from their scheduled appointment. If the Adopter fails to cancel the appointment at least 48 hours before the scheduled drop off time, they will be responsible for a no-show fee ($80).

After submission of this agreement, I will email a photocopy of the above ID to adopt@waggytailrescue.org.
