Required Transport Information and Documentation

Please fill out one form for each of the following: Pregnant dogs, Nursing dogs with Litters, Litters of 2 or more puppies (only those without mothers), Single dogs and/or puppies. For those sending multiple litters or dogs, a separate form will need to be filled out for each category!
Please be generous with size estimations. We especially do not want pregnant dams or moms nursing puppies to be in a space that is too small. The trip is long and we do not want anyone to get injured. Our van cannot accommodate many 42 inch and larger crates, so sometimes it is best to do private transports if many XL crates are needed. Large litters can be split into multiple crates. If this is the best space saving option, please choose more than one crate option to accommodate this litter. Nursing mothers with puppies under 6 weeks of age cannot be separated on transport.

Health and Wellness

We must consider the health and wellness of every single dog boarding the transport van. Dogs with any of the following will not be allowed to board: cough, runny nose, sneezing, watery diarrhea, or vomiting

Documentation

All Health Certificates and vaccination records MUST be submitted prior to 8:00 pm the night before transport, or the dogs you are sending will not be allowed to board.
Mothers with nursing babies only need one health certificate; however, all babies must be listed on the certificate (i.e. mother with 9 babies)
Pregnant dogs and dogs nursing puppies are not required to have any vaccinations. All litters and single puppies must have at least one set of vaccinations (DAPP) prior to boarding. Those coming from shelter environments must have a minimum of two vaccinations, no more than one month apart, prior to boarding. Adult dogs who are not pregnant or nursing must be up to date on all vaccinations (rabies, DAPP, bordatella)
