Adoption Contract - Dogs

I will provide a safe, loving environment for this dog and accept this dog as a member of our family. We will provide proper nourishment and medical care including monthly heartworm and flea prevention, as well as regular grooming. This dog will live inside my home, not outside.

I recognize that although PPR has made an effort to provide a dog which is compatible with our home environment, the dogs are in PPR foster care for a very short period of time. Therefore, PPR cannot and does not warrant the temperament or behavior of this dog. We release PPR and its members from all liabilities that may result from our acquisition of this dog.

PPR has advised me of any known health issues, however PPR strongly recommends that we take this dog to our veterinarian as soon as possible for a wellness exam and heartworm and flea prevention as the dogs are in PPR foster care for a very short period of time and could be carrying a virus or other illness that may be contagious to other animals.

I acknowledge that this dog may have tested positive for heartworms and if so was or is being treated for this disease and MUST be on Heartgard for at least one year. After one year we can keep the dog on Heartgard or switch to another brand. As a result, we acknowledge that this dog may test positive for heartworms for up to one year.

I agree that this dog will never leave the United States. If I have plans to move out of the country, I will contact the rescue immediately to make arrangements to return the dog back into PPR's care or discuss other rehoming options that must be approved by the rescue.

I will notify PPR immediately of any change of address.

I will immediately notify PPR if this dog is lost or stolen or if we are unable to keep this dog. We won’t relinquish this dog to another party without the written consent of PPR. This dog will wear a collar with identification tags at all times.

PPR is authorized to remove this dog from my home if it is judged not to be receiving proper care or if there is a violation of this agreement. We agree to permit PPR to visit the home where the dog is kept given reasonable notice and that such entry shall not constitute a trespass.

I agree to contribute a donation of PPR's choosing to PPR for the adoption of this dog thereby enabling the continued funding to support the program for the rescue of dogs. We understand that this donation is non-refundable, even if we return this dog. The adoption fees are listed on our Adoptable Dogs website or were communicated to the adopter prior to the adoption of this dog.

I acknowledge that the purpose of this agreement is to safeguard the health and well-being of this dog. I enter into this agreement freely and with goodwill.

I understand that I am adopting this dog for myself, and my home and not for another party. Rehoming this dog is not an option and if for any reason I am unable to keep this dog, I must notify PPR immediately to make arrangements for the dog to go back into the rescue's care.

I acknowledge that violating any terms of this contract may result in legal action that could result in fees and the return of the adopted dog.
