This form is a Surrender Request only. Completing and submitting this application does not guarantee that Best Paws Rescue will be able to accept your animal into our rescue. All surrender requests are reviewed on a case-by-case basis and are subject to foster home availability, the animal's needs, and our current capacity.

Forfeit of Rights

If Best Paws Rescue agrees to accept my animal into its program, I understand that by signing this document, I permanently surrender and forfeit all rights, title, and interest in the animal. I understand that I will have no further right to reclaim the animal, receive updates about the animal, or direct or dictate its future care, placement, or adoption.

Accuracy of Information

I certify that all information provided in this Surrender Request is true, complete, and accurate to the best of my knowledge. I understand that Best Paws Rescue relies on this information to evaluate the animal's needs, determine appropriate placement, develop any necessary training or behavioral plans, and ensure the animal's overall health and well-being. I understand that providing false, misleading, incomplete, or omitted information, or failing to fully disclose the animal's medical or behavioral history, may result in my Surrender Request being denied. I acknowledge that it is in the best interest of the animal to be completely honest and transparent throughout the surrender process.
