Volunteer Release Form

Please read carefully before signing. This is a release of liability and waiver of certain legal rights. 

In consideration for my being permitted to participate in training or other events organized by Bradford's Second Chance Rescue, I agree to the following Waiver and Release of Liability.

I acknowledge that handling or being near dogs, cats and any other animals where training or other activities occur, have inherent risks, hazards, and dangers for anyone that cannot be eliminated.  I UNDERSTAND THAT THESE RISKS, HAZARDS, AND DANGERS INCLUDE WITHOUT LIMITATION:

1. The propensity of an animal to behave in ways that may result in injury, harm, or death to persons on or around them;
2. The unpredictable nature of an animal’s reaction to such things as sounds, sudden movement and unfamiliar objects, persons, or other animals;
3. Certain hazards such as surface and sub-surface conditions;
4. Collision with other animals or objects;
5. The potential of the participant to act in a negligent manner that may contribute to injury to the participant or others, such as failing to maintain control over the animals or not acting within his or her ability
6. Possible exposure to contagious or zoonotic diseases

I understand the risks, hazards, and dangers described above. I understand that these activities may require good physical conditioning and a degree of skill and knowledge.  I believe I have that good physical conditioning and the degree of skill and knowledge necessary for me to engage in these activities safely.

My participation in these activities is purely voluntary.  No one is forcing me to participate, and I elect to participate in spite of the risks.  I AM VOLUNTARILY PARTICIPATING IN THE ACTIVITY ORGANIZED OR SPONSORED BY BRADFORDS SECOND CHANCE RESCUE WITH FULL KNOWLEDGE OF THE INHERENT RISKS, HAZARDS, AND DANGERS INVOLVED AND HEREBY ASSUME AND ACCEPT ANY AND ALL RISKS OF INJURY, ILLNESS, PARALYSIS OR DEATH. 

If less than eighteen (18) years of age, parent, guardian, or custodian must sign 

I hereby agree that I am providing volunteer services to Bradford's Second Chance Rescue, hereinafter referred to as the Rescue, including services such as assisting with pet adoptions, fostering companion animal(s), or working at special events. I acknowledge that my services are provided strictly on a volunteer basis, without any pay or compensation of any kind, and without liability of any nature on behalf of the Rescue (or Claus Paws Animal Hospital if volunteering for a surgery event). All services to be performed by me are at my own risk.  Agree I recognize that in handling animals and performing other volunteer tasks, there exists a risk of injury to me or my property, including but not limited to physical harm caused by the animals and/or transference of disease by the animals and I hereby expressly assume all such risks.  Agree Furthermore, I give permission for Bradford's Second Chance Rescue to broadcast live, make a video and/or audio tape, take photographs, generate computer images, or other images of me in various uses during volunteer activities for the Rescue. 

I HAVE CAREFULLY READ, CLEARLY UNDERSTAND, AND VOLUNTARILY SIGN THIS WAIVER AND RELEASE AGREEMENT.
