By submitting this application and/or volunteering with Jack Jack’s Pack Street Dog Rescue, I affirm that all information provided by me is true and complete to the best of my knowledge. I understand that as a volunteer, I do not have ownership or decision-making authority over any animal in the care of Jack Jack’s Pack Street Dog Rescue, and that all medical, behavioral, and placement decisions remain solely with the organization. I acknowledge that volunteering with an animal rescue involves inherent risks, including but not limited to bites, scratches, illness, injury, or damage to personal property. I voluntarily assume all such risks associated with my participation. I hereby agree to release, waive, discharge, and hold harmless Jack Jack’s Pack Street Dog Rescue, its directors, officers, volunteers, agents, and representatives from any and all claims, liabilities, demands, damages, costs, or causes of action of any kind, whether known or unknown, arising out of or related to my participation as a volunteer, including claims arising in whole or in part from the negligence of Jack Jack’s Pack Street Dog Rescue, to the fullest extent permitted by law. I understand that any false statements, omissions, or misrepresentations made by me may result in immediate termination of my volunteer role. I agree to promptly notify Jack Jack’s Pack Street Dog Rescue of any incidents, injuries, illnesses, escapes, bites, or concerns involving animals, people, or property that occur during my volunteer activities. By submitting this application or volunteering with Jack Jack’s Pack Street Dog Rescue, I acknowledge that I have read, understand, and voluntarily agree to the terms of this release.

I agree not to create or maintain any website, Facebook page, social media account, blog, or other online platform, nor participate in any public forum, including review or rating sites, for the purpose of slandering, defaming, or maliciously harming Jack Jack’s Pack or any of its current or former employees, volunteers, officers, directors, or board members by posting or sharing disparaging or false content.

I agree to use contact information provided by Jack Jack’s Pack solely for its intended purpose and to refrain from contacting volunteers, fosters, adopters, officers, directors, or other affiliated individuals for solicitation, disparagement of the organization, or any purpose that could reasonably be expected to harm the reputation of Jack Jack’s Pack.
