This application is intended for OWNED pets of Wood County Ohio residents only. Due to limitations, WCHS is unable to remove colonies of community cats. WCHS will not accept stray cats with "tipped ears" as strays or owner surrenders as it negatively impacts TNR efforts. Please reach out to us for tips for caring for cat colonies. 
Those named on the Surrender Application as the legal owner/care giver of the animal must be present at the scheduled appointment time. A completed application is not a guarantee of surrender availability. 

WCHS takes in surrenders as space allows, an application does not guarantee that you will be able to surrender your pet.  All intakes of strays and surrenders must pass a behavioral assessment before they can be accepted into our shelter. 

Surrender fees are due at the time of the appointment. Failure to complete forms, including image submissions, or surrender ID will be considered in an incompleted application. WCHS will not contact you to correct incomplete forms. 
Cats must be transported in appropriate cat carriers. Surrenders will not be accepted carried loose up to the door or in totes, baskets or boxes. 
Carriers are available for rental prior to your appointment if needed. 

Once an application is received, we will contact you to discuss options and if the application is approved and appropriate space is available, we will schedule a time to bring your animal to the Rescue for surrender.

Reason For Surrender

Please describe the reason you are looking to surrender the pet. Please note we cannot accept dogs with bite history.

We will ask for a surrender fee when you bring a pet to the shelter. In the case of stray cats, we do ask for a donation. Caring for the pet costs the shelter more money than earned in adoption and relinquishment fees combined. Current surrender fees are: $25 per cat $60 per dog
