TNR Agreement & Waiver

I agree to the following statements and also agree to adhere to all policies and procedures set forth by Rescue330.

• I understand the TNR Program is only for homeless community cats.
• If I choose to save or rescue any cats from a homeless and/or colony situation - they are not eligible for the TNR Program.
• I understand that representatives and/or volunteers from Rescue330 will be on my property to trap and return cats in the TNR Program. Rescue330 TNR volunteers are permitted to be on my property for the sole purpose of trapping and/or returning TNR cats.
• I understand that if i have any questions or concerns regarding the cats and their medical procesures, I will contact a Rescue330 representative; I will not contact the veterninary clinic myself. 
• I further agree if a cat that I saved or rescued has been serviced by the TNR Program and has been adopted or homed I am responsible for reimbursing Rescue330 for all services. I may be deemed ineligible for services if this occurs until the reimbursement has been satisfied.

I will adhere to the following key procedures in order to protect myself, Rescue330 staff and volunteers, and the cats I trap. I will:

• Bring feral cats to the clinic in humane cat traps only. 
(Not pet carriers, boxes, or collapsible traps.)
• Ensure each trap is lined with newspaper.
• Ensure that when a cat is inside a trap, the trap will be fully and completely covered AT ALL TIMES to minimize stress on the animal. Trap is also zip-tied to ensure cat cannot escape.
• I will not touch or handle community cats to avoid personal injury.
• I will never leave a trap out unattended, for any reason, or for any length of time.
• I will only trap the night before my appointment.
• I understand that cats need to be kept indoors in the humane trap the night before surgery.
• I will make sure that cats DO NOT have any food after midnight the night before their surgery.
• I will not transport cats in the back of an open bed truck, a sealed trunk, or a sealed trailer.
• If I am not able to transport the cats to and/or from surgery, I understand it is my reponsibility to request transport assistance from Rescue330.
• I understand that cats need to be kept indoors in the humane trap the night after surgery before being returned to their colony.
• I will follow all after-surgery instuctions from the veternarian.
• I agree that Rescue330 may contact me via email regarding my scheduled visit or other Rescue330 news.
• I understand that the cat's right ear will be "eartipped" for spay/neuter identification purposes.

This agreement and understanding may be updated, changed, or altered at the discretion of Rescue330 and/or its assigns. I understand I may not be updated via email and may be updated through other processes to include writing and verbally either in person or on the phone.

I give my permission to the Rescue330 to verify the above information. I understand that this application does not guarantee acceptance to the Rescue330 volunteer program.

I, hereby agree to accept a position as a volunteer for Rescue330, and in doing so, I agree to comply with all of the rules and regulations which may be established from time to time by the Rescue330, and I understand that failure to do so may result in my immediate termination as a volunteer.

I understand that I am held to a level of confidentiality regarding personal information about adopters or other matters that fosters, volunteers, and board members may discuss in meetings and via email.

I grant Rescue330 the use of images of myself or likeness in connection with the volunteer experience for the benefit of the organization. Images can be used on Rescue330’s website, social media, newsletters, and other advertising.

I acknowledge that my services are provided strictly on a volunteer basis, without any pay or any kind, and without liability of any nature on behalf of the Rescue330. All services will be performed at my own risk. I recognize that in handling animals and performing other volunteer tasks there exists a risk of injury, exposure to zoonotic diseases (i.e. ringworm, giardia, rabies, etc.), and physical harm caused by the animals. On behalf of myself, my heirs, personal representatives, and executors, I hereby release, discharge, indemnify and hold harmless the Rescue330, its volunteers, affiliates, members, board of directors, and employees from any and all claims, causes of action, or demands, of any nature or cause, including costs and attorney's fee incurred by the Rescue330 in connection with the same, based on damages or injuries which may be incurred or sustained by me in any way connected with my services for the Rescue330, including but not limited to animal bites, accidents, or injuries.

I HAVE READ THIS VOLUNTEERING AGREEMENT, RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
