Adult Spay/Neuter Agreement – MLRR

As part of the adoption process, I understand and agree to the following terms regarding the spay/neuter of my adopted dog from MLRR:

MLRR-Arranged Appointment:
MLRR has scheduled a spay/neuter appointment for my dog with an MLRR-approved veterinarian. The adopter is responsible for getting the dog to and from the veterinarian. 

MLRR will cover the cost of the procedure if I bring the dog to this pre-arranged appointment at the date and time communicated to me by the MLRR Medical Team.

If I Miss the Scheduled Appointment:
If I am unable to bring my dog to the scheduled appointment:

I agree to have the dog spayed or neutered by a licensed veterinarian at my own expense.

This procedure must be completed by the time the dog is 12 months of age.

Once I submit written proof of spay/neuter, MLRR will provide reimbursement as follows:

$130 for neuter

$150 for spay

Non-Compliance:
I understand that MLRR reserves the right to reclaim any adopted dog if I do not comply with the spay/neuter requirements outlined above. MLRR is not responsible for getting the dog to and from the vet for the surgery.

Additional Agreements:
I confirm that I have also completed and signed the MLRR Adoption Agreement and that I understand and agree to all terms stated in both the MLRR Adoption Agreement and MLRR Adoption Policies.

I understand and agree to the terms of this adoption agreement attachment.
