Thank you for taking the first of three steps in enrolling your cat in the June’s Senior Cat Rescue Feline Legacy Program. Our program is designed to ease the transition from a cat’s familiar home into our care after the death of their owner. We want you to rest assured knowing that by joining our family of Feline Legacy Program enrollees, your cat’s happiness, comfort, and safety will be our highest priority. To begin the process of enrolling your cat(s) in the Feline Legacy Program, please answer the following questions about your cat(s). If you have any questions, please call us to discuss them at 505-220-3528. Finally, please email an image of your cat to us at info@jscr.orgOnce you have filled out this form, we will be in touch for the second step: learning as much as possible about your cat, especially regarding behavior and medical issues. This information will help us to support your cat(s) in adjusting to their life without you. The third, and last, step is signing an agreement form giving us guardianship of your cat if you become incapacitated or pass.

Does your cat fight with another cat in your household? If yes, please provide the name of your cat’s nemesis and describe the relationship.
If you answered yes to the above question, have you previously tried any behavior modification remedies or medications to try to resolve your cat’s differences with the other cat? If Yes, what did you try, when did you last try it, and what were the results?
If you answered yes to the above question, do you keep these cats separate in your home now? If Yes, please describe the living arrangements
