Direct Adopt Agreement

Please list the name of the MLRR dog you are Direct Adopting

I agree to follow the MLRR guidelines in this agreement and agree to provide a $50 donation to be used toward the dog's adoption fee.  I understand that this will be a donation to MLRR should I choose not to adopt the dog at the end of the two-week wellness period.

An MLRR identification tag will be provided by MLRR, and I will attach it to the dog’s collar immediately. The tag will remain on the dog’s collar at all times.
I will provide the dog with comfortable, clean, indoor housing, a premium food diet, and fresh water available at all times. I agree to keep the dog in a secure, fenced environment or on a leash at all times when he/she is outside of my home. The Lab will sleep inside my home. Furthermore, I agree to never use electric training devices or invisible fencing with the dog.  I will only use positive, force-free training techniques.

Veterinary expenses during the two-week wellness period, including but not limited to vaccinations, heartworm testing, heartworm preventative, fecal testing, and spaying/neutering will be covered by MLRR. All such treatments (except for emergency situations) must be APPROVED IN ADVANCE by the MLRR Medical Coordinator. Unapproved expenses will not be reimbursed.  Additionally, I understand the treatments must be performed at an MLRR approved vet.  If I choose to go to a veterinarian that is not on the MLRR approved veterinarian list, then I understand MLRR will reimburse me for approved expenses at MLRR's rescue rates.  

In the event of a medical emergency, I will notify the MLRR Placement Coordinator or MLRR Medical Coordinator as soon as the dog is safely at the veterinarian or sooner if I need direction on how to handle the emergency situation.

In the event that the dog bites a human, I will notify the MLRR Placement Coordinator or a MLRR Board Member immediately. I will disclose all information regarding the bite incident.

I understand that the MLRR Board of Directors will make all decisions regarding any additional medical or behavioral evaluations or treatments of the dog in my care.

Receipts for approved expenses should be attached to MLRR's online Reimbursement Request Form and submitted within 60 days of the expense.

If there is any change in the dog’s health, personality or eating habits, I will contact the MLRR Placement Coordinator or MLRR Medical Coordinator immediately.

I will notify MLRR immediately if the dog is lost or stolen, and I will make all reasonable efforts to locate and retrieve the dog.

I will assume liability for the dog while it is in my care.

I will not sell or give the dog to anyone unless approved by the MLRR Board.

I will adopt the dog in my care during or at the end of the two-week wellness period. That time is extended for dogs with extensive medical needs (like heartworm) since treatment times are typically longer. Once the treatment is completed, I will complete the online Adoption Agreement and submit the adoption donation per the online instructions. The adoption will then be finalized.

I will receive reimbursement of my initial $50 deposit within 2 weeks of submitting the full adoption donation amount.

I understand that MLRR will retain ownership of the dog until it is adopted. I will surrender the dog to MLRR immediately if requested to do so by a Board member for any reason at any time.

I understand that all MLRR supplies will be returned to MLRR upon request. If these items are not returned I am responsible for reimbursing MLRR for all supplies.
