All puppy wellness visits and vaccinations after the date of adoption will be at the cost of the adopter. It is recommended that you, the adopter, follow up with your veterinarian post-adoption for a wellness visit. At that time, your veterinarian can review the schedule of vaccinations that your puppy will need. Puppies are typically vaccinated at 6 weeks, 9 weeks, 12 weeks, and 16 weeks. A one-year rabies vaccination is typically given between 12-16 weeks of age. I agree that all medical expenses going forward are my responsibility. I understand that some conditions may not have appeared during the time that the above-mentioned dog was in the care of MLRR. I understand that some illnesses, such as kennel cough or fecal parasites, can lay dormant for several weeks. My new puppy will not be left alone for a “normal” business day. I will make arrangements so that my puppy will not be left alone for more than 4 hours. At a minimum, someone will take the puppy outside for a “potty” break in the middle of the day. If my new puppy is under 5 months old, I will enroll her/him in a puppy socialization class that uses only force-free positive training techniques immediately after adoption. A puppy socialization class typically provides an opportunity for socialization with other puppies, people and items in a safe and controlled environment with guidance from a knowledgeable trainer during a critical developmental age.
I will be required to provide a $50 deposit for spay/neuter of my new puppy at the time of adoption if she/he is not yet spayed/neutered. This is in addition to the adoption fee.
MLRR-Arranged Appointment:
MLRR will schedule a spay/neuter appointment for my dog with an MLRR-approved veterinarian. The adopter is responsible for getting the dog to and from the veterinarian. MLRR will cover the cost of the procedure if I bring the dog to this pre-arranged appointment at the date and time communicated to me by the MLRR Medical Team.
If I Miss the Scheduled Appointment OR If I am unable to bring my dog to the scheduled appointment: I agree to have the dog spayed or neutered by a licensed veterinarian at my own expense. This procedure must be completed by the time the dog is 12 months of age.
Health Exceptions:
The 12-month spay/neuter deadline may be extended only if I provide MLRR with a written statement from a licensed veterinarian indicating that the procedure would pose a health risk to the dog at that time.
Non-Compliance:
I understand that MLRR reserves the right to reclaim any adopted dog if I do not comply with the spay/neuter requirements outlined above. MLRR is not responsible for getting the dog to and from the vet for the surgery.
Additional Agreements:
I confirm that I have also completed and signed the MLRR Adoption Agreement and that I understand and agree to all terms stated in both the MLRR Adoption Agreement and MLRR Adoption Policies.
If for any reason I cannot keep this puppy, I will not transfer ownership to any other person or shelter before contacting MLRR.
In addition to this signed contract attachment for my puppy, I have completed and signed an MLRR Adoption Agreement. I understand and agree to all of the information in the MLRR Adoption Agreement and in the MLRR Adoption Policies.
