This form is to be completed by people in the Omaha metro area requesting Stray to Spay's help to trap, neuter, vaccinate, and return community cats (both feral and stray cats).

This form needs to be completed by the people/person whose property is the site where the cats are located, or filled out on their behalf. Please do not use this form if you do not have permission from a property owner to TNVR cats on their property.

Stray to Spay Protocol

Stray to Spay’s program includes working with community members to help them trap cats. We are a small organization and priority is given to people who can set and monitor traps. If there is a reason you cannot set and monitor traps, we will do our best to fit you in when we can.

Cats that go through Stray to Spay's program receive an ear tip on the cat's left ear, sterilization, and a rabies vaccine. Cats also receive a general wellness check for obvious health issues such as open wounds, abscesses, etc. and will receive treatment accordingly.

I RELEASE STRAY TO SPAY, AND ITS VOLUNTEERS, FROM ANY LIABILITY INCURRED WHILE TRANSPORTING OR CARING FOR THESE CATS.I AGREE THAT EACH CAT WILL BE SPAYED/NEUTERED, LEFT EAR TIPPED, VACCINATED FOR RABIES, AND RETURNED BACK TO WHERE THEY CAME FROM.I AGREE THAT ANY CAT DEEMED BY THE VETERINARIAN TO BE SEVERELY ILL OR INJURED WILL BE HUMANELY EUTHANIZED. BEST EFFORTS WILL BE MADE TO CONTACT ME FIRST. FERAL CATS FACE RISKS DURING HANDLING, ANESTHESIA, AND SURGERY AND I HOLD STRAY TO SPAY AND ITS VOLUNTEERS HARMLESS SHOULD A CAT EXPERIENCE COMPLICATIONS, INJURY, ESCAPE, OR DEATH. IF I CANNOT BE REACHED AT THE NUMBER GIVEN, STRAY TO SPAY WILL MAKE THE DECISION TO HUMANELY EUTHANIZE IF DETERMINED TO HAVE A CONDITION MAKING ITS RELEASE INHUMANE.I AGREE THAT IF A CAT TESTS POSITIVE FOR THE FELV VIRUS, THE VETERINARIAN HAS THE RIGHT TO HUMANELY EUTHANIZE THE CAT AT THEIR DISCRETION.I UNDERSTAND THAT DUE TO THE LIMITATIONS OF THE TNR PROGRAM, OTHER THAN STERILIZATION AND RABIES VACCINATIONS, NO OTHER VETERINARY SERVICES WILL BE PROVIDED OUTSDIE OF MEDICALLY NECESSARY EXCEPTIONS AS DEEMED BY THE VETERINARIAN AND AT THE DISCRETION OF STRAY TO SPAY.I AGREE TO RETURN ALL EQUIPMENT, INCLUDING BUT NOT LIMITED TO: TRAPS, TRAP COVERS, AND TRAP DIVIDERS.I AGREE TO KEEP ANY APPOINTMENTS MADE AND IF UNABLE TO DO SO, I WILL GIVE AT LEAST 24 HOURS NOTICE IN ADVANCEI UNDERSTAND THAT CATS WILL NOT RECEIVE ANY ADDITIONAL SERVICES BEYOND SPAYING/NEUTERING UNLESS DETERMINED BY THE VETERNARIAN AND I AGREE NOT TO REQUEST ANYTHING ADDITIONAL (INCLUDING BUT NOT LIMITED TO NAIL TRIMS, FLEA MEDS, SHAVES, ETC.)I ACKNOWLEDGE THAT COMPLETION OF THIS FORM DOES NOT GUARANTEE THAT STRAY TO SPAY WILL BE ABLE TO PROVIDE IMMEDIATE ASSISTANCE AS SERVICES ARE BASED ON FIRST COME, FIRST SERVE BASIS AND AVAILABLE FUNDING FOR VETTING APPOINTMENTS.
