FRIENDLY STRAY Surrender Form

This form is for cats and kittens that are considered FRIENDLY STRAYS (likely someone's pet, friendly, sometimes shy). These cats may run away from you but will soon allow you near them. Or they will come on your property and try to get in your home. They can be lost or possibly dropped off by their owner. Some adults will have a litter of kittens with them. We need to contain both the kittens and the mother. The only exception is when the mother cat is verified deceased.

Please email clear photos of the cat(s) to fur.all.kittys@gmail.com, our Facebook messenger or text them to 412.979.9855. Also, please send videos to Facebook messenger if possible. Photos and videos help us to get an idea of the cats conditions, how many there are, and a feel for the area that we will be trapping. When texting, please provide your name and why you are texting.

Fill out the form with as much information as possible so that we can come up with the best solution for the animal. Failure to prvide details and answers to questions may result in you not receiving a response.There is no identified time frame offered for a response about this form It is necessary because we receive numerous requests daily and are all volunteers. Please indicate if your situation is urgent by sending a text. 

Fur All Kittys (FAK) does not promise or guarantee that we will be able to assist you with your needs. When we cannot help, we will make every effort to provide additional resources or contacts (with no guarantee on outcome). 

There is a suggested donation with any request. The animals that need care cost money to vet, spay/neuter, feed and shelter. Donations can be cash or some type of needed items. We know that these are not your cats, but they are not ours either and we must find a way to cover costs associated with taking them into the rescue. None of the vet services we obtain are free to us and we pay office and other fees just like any person does. 

Some strays are easy to catch and contain. Others may not be depending on how long they have been outside. Please understand that our time is valuable (and we are volunteering) and we may need your assistance to contain the cat(s). We may also need you to bring the cat(s) to the rescue in West Mifflin. If you are not able to help with any of this, it is going to take longer to assist you.

Please provide as much detail as possible on the cats in question. We need to know what type of situation we are dealing with. Several times, there are kittens involved and that requires different planning. We will often need their mother to continue their care until they are old enough and this requires knowing which cat is the mother. It is very important to have an idea on numbers and health of the cats ahead of time if possible. 

Please read the following statements. At the end you will be asked to sign in agreement with all of the following. 

• All of the above information is true to the best of my knowledge. I understand that providing false information could impact any future afiliation with Fur All Kittys.
• I attest that the cat(s) being surrendered are not my personal cats and I have not taken the cat(s) from any owner maliciously. 
• I am choosing of my own volition to surrender the stray cat(s) to the rescue, Fur All Kittys. 
• The rescue is not expected to provide me with any additional information on this animal; however I may contact them for updates with the understanding that I may not receive them. 
•  I understand that after the surrender, I may not request the return of the cat(s). 
• I understand that if I wish the cat(s) to be returned to my care, I will proceed with the adoption process established by the rescue. I understand that even in doing this, I may not be approved for the cat(s) for any reason outlined in their process. I will not dispute the decision made regarding my application.
