I hereby certify that I am the rightful owner/keeper/caretaker/custodian of the animal(s) who is/are the subject of this Return form, herein after referred to as “the animal.”

I hereby surrender any and all property rights to the animal.

I certify that no other person has a right of property to the animal.

I understand that if I adopted this animal 14 days ago or less, I will receive a full refund of the adoption fee, less a $25 processing fee.

If it has been 15 days or more since the date of adoption, I will not receive a refund at all.

I understand that by surrendering my property rights to the animal, the animal will become property of Angel of Hope Animal Rescue and will not be returned to me.

I also hereby certify that the animal has not bitten or scratched a human or another animal within the past 10 days.

I have disclosed any behavioral and health issues for the listed animals.

I further certify that I have read and understand the terms of this Return Form.
