As the parent or legal guardian of the minor named above (referred to as “the Volunteer”), I hereby acknowledge and agree to the following:

Assumption of Risk
I understand that participating in a dog rescue event involves inherent risks, including, but not limited to, injuries caused by animals, slips, falls, or other accidents. I accept full responsibility for any injury, harm, or loss that may occur to the Volunteer while participating in the event.

Release of Liability
I hereby release and hold harmless Happily Ever After Rescue (H.E.A.R.), its staff, volunteers, board members, agents, and representatives, from any and all claims, actions, or liabilities arising out of or connected with the Volunteer’s participation in this event, including but not limited to claims for injury, property damage, or other losses.

Medical Treatment Authorization
In the event of a medical emergency, I authorize H.E.A.R. to provide or seek medical attention for the Volunteer. I understand that I will be responsible for any costs incurred for such treatment.

Photograph and Media Consent
I consent to the use of photographs, videos, or other media taken during the event for promotional, educational, or fundraising purposes by H.E.A.R. I understand that no personal identifying information will be shared.

Code of Conduct
The Volunteer agrees to abide by all rules, guidelines, and instructions provided by H.E.A.R. staff and volunteers. Failure to comply may result in dismissal from the event.
