Where was the cat found? ***IMPORTANT NOTE*** If the cat is a FRIENDLY STRAY it could be a lost or free roaming pet. If possible, please take it to a local vet to get scanned for a microchip prior to requesting intake to see if an owner can be located. Any nearby vet will perform a microchip scan free of charge.
Please provide information regarding the cat (i.e. cat or kitten, nursing mom, injuries/medical issues, friendly, shy, aggressive, how long you have seen it in the area, etc.)
We often have to decline requests due to no foster space. Would you be willing to foster the cat(s) for us? We provide food, supplies, and all medical expenses.

If "No", please provide information regarding owner's reason for surrender.
If "Yes" please detail the reason for wishing to surrender/rehome the cat and any interventions you have tried. If the reason is behavioral please note if you would be interested in us providing suggestions for additional interventions before surrendering.
Is the cat up to date on all vaccinations (Rabies, FVRCP)
If "no" are you willing to get them vaccinated prior to surrender or donate towards the expense of their vaccinations?

WE ARE A 5013C NON PROFIT ORGANIZATION THAT SURVIVES STRICTLY ON DONATIONS. AVERAGE INTAKE COSTS FOR EVERY CAT/KITTEN RUN FROM $150-$200. DONATIONS ARE DEFINITELY NOT REQUIRED BUT WOULD CERTAINLY BE APPRECIATED!
