I understand my application is a request for and not a guarantee of services.

I agree to be punctual in both drop off and pick up of mama cat before and after surgery.

If I intend to keep a kitten, or have a friend/family member wanting to keep a kitten, I understand that they are required to apply to adopt this kitten and pay the adoption fee after it has been spay/neutered and vaccinated.

I understand that all medical procedures come with the risk of complications and release Team Kitten, their vets, volunteers, and agents from any claims that may arise out of, result from, or are related to medical care received pursuant to this device. I am welcome to perform a health panel at my own cost before a procedure is performed but understand that the completion of an external health evaluation may reduce, but does not eliminate, the chance of complications arising.
