Owner Surrender Request

We understand that rehoming your dog is a very difficult decision and will do all we can to help you. Please fill out this form to the best of your ability. Your answers will help us get to know the animal in question, and will ensure that if we are able to place the dog in a foster home or a new home, that the home is adequate for the needs of the dog. Above all, please be honest  - failure to be honest with us can cause multiple issues, from the dog being placed in an incorrect foster to legal issues for you and our rescue. We will do all we can to help, but cannot make any promises that information entered on this form will result in the dog being taken into our program. In addition, if there is anyway you are able to keep your pet need assistance please let us know what assistance would be needed for you to keep your pet.

Surrender Agreement:  I certify that I am the owner of or have the authority to surrender the animal described to Second Chance Animal Rescue.  I hereby relinquish all rights of ownership and any right to information on the disposition of the animal described.  I also authorize the release of any veterinary records regarding the animal.  I certify that to the best of my knowledge I have disclosed all of the information about the animal concerning health, behavior, history and anything else that may affect the safe placement of the animal in a new home.  I also agree to indemnify and hold harmless Second Chance Animal Rescue, its employees, agents, volunteers or representatives, from any and all liability arising out of or in consequence of, any injury sustained while Second Chance Animal Rescue is assisting me with this animal.
