Creating an account or logging in via facebook/email will let you save your application, and allow you to access your pets medical records and other important information after you adopt or foster.
A limit of two (2) foster pets may be fostered at the same time by the same person. There may be exceptions as approved by PNH for example a mother dog with litter of puppies or sibling puppies, or an emergency foster situation. I consent to PNH doing home checks prior to and during the care of the foster pet. I will notify PNH via phone and email immediately if I am changing residences so that a new home inspection may be completed at my new address in advance of the move.
The foster pet will be under my supervision always. The foster pet will not remain overnight in the care of anyone other than myself, an immediate adult family member at the same address, veterinarian, or a PACFA licensed boarding facility. The foster animal being housed anywhere other than the address provided at the time of the home visit will be grounds for immediate termination of the foster contract and the foster dog will be immediately returned to a staff member.
I will bring my foster to a minimum of two adoption events per month and will continue to bring my pet to as many adoption events as possible until my foster pet is adopted. (Does not apply to foster to adopt families) I will notify the adoption event coordinator if I need help transporting my pet to or from an event or appointment. If events are not available, I agree to transport my foster dog to a neutral location for meet and greets with potential adopters and consider safety for all parties during such situations including a muzzle (provided by PNH and trained a minimum of 3 days prior with guidance) if necessary. (Under vaccinated puppies are excluded from the neutral location) I will provide photos and bio information as needed to help my foster get adopted. I agree to help "market" my foster by posting on Petfinder with rescue login, sharing photos and bio information.
If the foster pet is a dog, I agree to keep it on leash always while outside my home. I will not allow the dog to roam freely at any time, even for potty breaks. I will NOT take my foster pet to any play groups or dog parks. Going to Camp Bow Wow, working with a trainer, or coming to a PNH adoption event are the exceptions. If the foster pet is a dog that will go to work with me, I agree to ensure the dog behaves appropriately in an office setting. I agree not to allow visitors access to my foster to avoid fear based biting. I understand that I am responsible if my foster harms a person or another animal in any way and will take full responsibility, including attorney fees to bring the animal back to PNH.
I will read and familiarized myself with the foster pet's file. I will understand the foster pet's specific needs and have been educated on the proper ways of working with this foster pet's special needs. If the foster pet requires behavioral training, I will cooperate with PNH in scheduling and coordinating training sessions. I will assist in POSITIVE TRAINING METHODS ONLY based on American Kennel Club (AKC) Canine Good Citizen (CGC) or AKC Puppy Star for basic household manners. I will review all AKC authored CGC videos on YouTube and practice these with my foster animal.
I understand that all foster pets shall be provided with a secure area, including shelter from the elements. Foster pets are not to be kept tied or chained. All foster pets should be indoor family pets. Foster pets will not remain in cars or yards unattended or unsupervised for extended periods of time, especially during cold or hot weather. Foster pets should be transported in approved kennels or wear seatbelts when in a vehicle. Absolutely no animals are allowed to ride in the back of a pick up truck whether tethered, loose or crated. We expect all animals to ride inside vehicles for their safety.
I will call PNH at 719-500-1880 for veterinary care should the foster pet become ill or injured. I will notify PNH immediately via phone and email if my foster animal becomes lost or stolen. I will take the foster animal to all scheduled vet appointments (spay/neuter, etc.) or notify PNH within 48 hours if I cannot keep a scheduled vet appointment. In the case of foster, we REQUIRE spay/neuter appointments at PNH discretion. While veterinarians may disagree with early spay/neuter, it is absolutely safe and will be done in order to accomplish adoption per PACFA and eliminate the possibility of future unwanted litters. If a foster refuses to comply with the spay/neuter appointment, the dog will be relinquished back to PNH immediately and foster to adopt approval will be rescinded. Except in case of emergency, I will obtain PNH approval for obtaining veterinary care at a Pre-Approved Veterinary Clinic. PNH will be responsible for the cost of care ONLY at a Pre-Approved Veterinary Clinic with approval from the foster coordinator or director. When after surgery care requires a cone or special handling, I understand that I will be financially responsible for veterinary bills when I do not follow after care instructions.
The representations contained in the foster application are incorporated herein by reference and PNH is relying on those representations in entering into this agreement. This agreement is the entire agreement between PNH and the foster parent, superseding any prior understandings between the parties with respect to the subject matter of this Agreement. No modification of this agreement will be valid unless in writing and duly signed by PNH and the foster. This agreement is binding upon the heirs, assigns, successors, personal representatives, and executors of both parties. If I wish to terminate this agreement, I will notify PNH immediately to arrange for the foster pet's return to PNH. I understand that there may be a waiting period while we find another foster, sometimes up to 2 weeks. I agree to be financially responsible for any fees incurred to bring foster animal into a PNH approved home or facility.
I will ensure compliance with all applicable local ordinances, zoning and HOA and state statutes regarding possession of a foster dog or rules of ownership of Pet. If I am charged with violating any such ordinance or statute while in possession of foster pet, I understand that PNH is not responsible for obtaining legal counsel for me or for paying any fines or penalties assessed.
I hereby release PNH, its Board of Directors, and volunteers, and veterinarians, from any claim, cause of action, or liability for any injury or damage to self, persons, or property once the pet is in my foster care. PNH does not guarantee the health or mental health/behavior of any foster pet and is not financially responsible for any illness or injury caused to current pets in household or any other persons, pets, or property.
I acknowledge that the foster pet remains the property of PNH. If I fail to return the foster pet to PNH at the end of the foster period, PNH will notify me by phone first of the reclamation and ask that the animal be returned. If the animal is in such a circumstance to endanger its health, well-being, or life, PNH may recover the pet immediately or call animal control authorities to recover the animal on behalf of PNH. Any decision of PNH is final as to whether the pet will be returned to me.
Any violation of the terms of this agreement may be grounds for immediate termination of said agreement, and the foster pet shall be immediately surrendered to PNH. This agreement may be terminated with or without cause at the sole discretion of PNH.
By completing this form, I attest that I am a legal adult over 18 years of age, and that I understand and agree to the following: I agree to provide a foster home for assigned pet. I agree to notify the organization immediately before I take another animal into my home. I agree to keep my personal animals current with all vaccinations as recommended by the AVMA or my veterinarian. Rabies vaccination and proof of adequate DA2PPV is mandatory. I agree to keep a tag with my own contact phone number on the foster pet for the entire foster period. If provided, I will also keep a PNH Identification tag and/or microchip tag on the pet.
PNH no longer allows a payment plan for adoption when fostering. Dogs will be adoptable after the 14 day quarantine or after spay/neuter appointment. PNH is unable to hold dogs after their adoption availability has passed. Please expect to adopt the day the animal becomes adoptable when you have the animal on hold for adoption.
I will familiarize myself with the foster manual provided via email. I will also create a Petstablished account so that I am able to create a bio for my foster dog, have access to all intake and medical records and add any notes to help my foster dog get adopted into a suitable, permanent and loving home.
I agree and understand that supplies are kept at My Storage at Jackson Creek 1187 Harness Rd Monument, CO 80132. The codes will be provided to me. If there is something my foster needs that is not located in storage, PNH will order and or supply to you via pick up at a store location, a PNH drop off home or store pick up. When my foster assignment is complete, I agree to return all supplies, excluding used food, to this same storage locker and place it appropriately. I understand that I may choose to supply food, toys, etc to my foster animal, especially in cases of foster to adopt. PNH will not reimburse for expenses unless the expense has been approved in advance.
