Volunteer Application and Contract

I certify that the statements made in this volunteer application are true and have been given voluntarily. I understand that this information may be disclosed to any party with legal and proper interest, and I release the agency from any liability whatsoever for supplying such information. I also understand that I will not be paid for my services as a volunteer. In consideration of Native Nation and Animals Called to Service accepting my application or my child's for participation in their volunteer programs, I agree to release and hold harmless the Native Nation and Animals Called to Service from and against any and all loss, damage, claims, liability, costs, and expenses of any nature whatsoever, including without limitation, attorney's fees and disbursements, arising from or occasioned by my participation in the Native Nation and Animals Called to Service’s volunteer programs. I understand there are certain risks inherent in handling the animals, and I accept these risks. I agree that the Native Nation and Animals Called to Service may photograph my participation in this program, and I hereby release any such photographs to Native Nation and Animals Called to Service for use in its programs, publications, and purpose. Further, I understand that it is a violation of privacy and breach of confidentiality to publicly discuss or disclose any information I may learn about an animal’s previous owner or situation. I will treat all animals, people, and property I come into contact with at ACTS with the utmost respect and professionalism. I understand that difficult decisions and situations may present themselves routinely at ACTS, and I will conduct myself with courtesy and respect toward staff, volunteers, and the public at all times.

I agree to hold absolutely confidential all information that I may obtain concerning patrons or animals. I understand that an intentional or unintentional violation of confidentiality may result in disciplinary action, including termination of volunteer services and/or possible legal action by others (i.e. patrons). I fully understand that the ACTS Animals Called to Service program expects high standards of moral and ethical treatment of animals under its care. I agree to adhere strictly to these standards in my voluntary capacity and exercise compassion, care, and positive training methods with all animals. I agree to treat patrons and other guests of ACTS with courtesy, sensitivity, and professionalism. I will seek management help for any questions or situations beyond my knowledge or training. I agree to advise my supervisor of relevant information regarding an animal or patron in a timely manner. I will communicate job-related problems, concerns, differences of opinion, conflicts, or suggestions to the Volunteer Coordinator. I agree to limit my activity to my assigned work area unless otherwise directed by the Volunteer Coordinator or Shelter Manager. I agree to adhere to my schedule and sign-in and out procedures. I will try to find properly trained replacement for my scheduled shift should I be unable to make it. I will notify my department manager when unavailable for my shift. I agree to maintain appropriate volunteer attire and a well-groomed appearance for all assignments. I understand that my volunteer service is an integral part of the daily operations of the shelter and I agree to notify the Volunteer Coordinator immediately of any schedule changes or if I wish to discontinue my volunteer services. I understand that ACTS reserves the right to terminate my volunteer status at any time, for any circumstances which, in the judgment of the Volunteer Coordinator, Shelter Manager or Executive Director, would make my continued service as a volunteer contrary to the best interest of the agency. I have read and understood each of the above conditions. By checking the "I Agree" box below, I agree to adhere to the stated volunteer agreement.

The applicant acknowledges that services provided to ACTS are strictly voluntary and in a volunteer capacity without any express or implied promise of salary, compensation or other payment of any kind. The applicant further understands that certain risks may be associated with performing these services and he/she accepts those risks. In consideration of being permitted to perform volunteer services for the ACTS, the applicant, for himself/herself and his/her heirs and representatives voluntarily and knowingly executes this document and expressly waives any and all rights, claims or causes of action, including, without limitation, those involving bodily injury or property damages to the applicant, or to the applicant’s family or property while the applicant is engaged directly or indirectly, in performing services, whether or not caused by the negligence of ACTS, its officers, board of directors, agents or employees. The undersigned hereby agrees to indemnify, defend and hold ACTS, its officers, board of directors, agents and employees harmless from and against any and all liability, damage, loss, cost and expense incurred as a result of any claim, demand, or cause of action, brought against ACTS, its board of directors agents or employees, jointly or individually, for bodily injury or property damage suffered as a result of the applicant’s reckless or willful act or omission in the performance (or failure to perform) of volunteer services. By checking the "I Agree" box below, the applicant has read and fully understands the contents of this Waiver of Liability. This Waiver of Liability shall continue in full force and effect until all-volunteer services have been terminated.

By checking the "I Agree" box below, I hereby grant permission to the ACTS to use my likeness in photograph(s) and/or video in any and all of its publications and in any and all other media, whether now known or hereafter existing, controlled by ACTS in perpetuity. I will make no monetary or other claims against ACTS for the use of the said photograph(s) and/or video.
