Volunteer Application

We are so pleased and greatful that you have chosen our rescue to donate your time to. This form is to be filled out by a legal guardian of the minor applying to volunteer. By submitting this form, you understand and agree to all conditions and statements below.

 I, the undersigned, volunteer my time and services to assist L.O.L.A.’S Rescue (“LR”) because of my support for LR and my desire to actively participate in the furtherance of LR’s charitable mission. In consideration of the opportunity to participate as a volunteer for LR, I hereby agree as follows: 

1. Voluntary Participation. I fully understand and agree that I am providing my services to LR as a volunteer. As a volunteer, I will not be paid for my services and I will not be covered by or eligible for LR insurance, worker’s compensation, or other benefits. My participation with LR may be terminated at any time by LR or myself. 

2. Policies and Procedures. I agree to abide by all LR policies and procedures as currently existing and as may be updated from time to time. 

3. Assumption of Risk. I understand that the nature of the volunteer activities I may perform may involve physical activity, contact with unidentified or unfamiliar persons or animals, and potential risk of physical injury or damage to property. I expressly assume full and complete liability for any injury, loss, or accident I cause that may occur during my volunteer work with LR. 

4. Release of Liability. BECAUSE THE ASSERTION OF CLAIMS AGAINST LR WOULD BE CONTRARY TO MY SUPPORT OF LR AND ITS GOALS AND WOULD REDUCE THE ABILITY OF LR TO ACCOMPLISH ITS CHARITABLE PURPOSE, TO THE FULLEST EXTENT OF THE LAW, I, MY NEXT OF KIN, MY HEIRS, ADMINISTRATOR, AND EXECUTORS HEREBY RELEASE AND HOLD HARMLESS LR, AND ITS AGENTS, SERVANTS, INSURERS, AND EMPLOYEES FOR ANY INJURY, LOSSES, OR DAMAGES I MIGHT SUFFER IN CONNECTION WITH MY VOLUNTEER WORK FOR LR, REGARDLESS OF THE CAUSE THEREOF. 

5. Confidential Information. I understand that as a volunteer for LR, I may have access to sensitive or confidential information. I agree to maintain the confidentiality of all information I receive in connection with my volunteering for LR including, but not limited to, all information regarding actual or prospective recipients of  LR services such as identity, address, contact information, health and disability information, and financial information as well as information about LR’s business operations, such as organizational structure, employee information, financial operations, marketing strategy, donor lists and amounts, plans for upcoming events, current or proposed business transactions and sponsorships, and any proprietary information such as computer software and programming. 

6. Intellectual Property. I hereby irrevocably assign to LR the entire right, title, and interest in and to any and all ideas, methods, inventions, software, documentation, utilities, tools, devices, improvements, and any and all deliverables and work product, whether or not copyrightable or patentable, originating with, conceived, invented, acquired or developed, either solely or jointly with others, in connection with my volunteer services.  In addition I agree to pay all legal fees associated with LR retrieving said information and damages associated if property cannot be retrieved or restored to its exact original form.

7. Media Release. I consent to the unrestricted use by LR, its agents, and others working under its authority, of any photographic portraits, pictures, video footage and/or audio recordings in connection with my participation as a volunteer (collectively, the “Material”). LR has the right to reproduce, exhibit, distribute, broadcast, digitize, edit, or otherwise use the Material without restriction throughout the world, including by incorporating the Material into its website, publications, catalogues, brochures, books, magazines, or commercial, informational, educational, advertising, or promotional materials relating thereto (collectively, the “Works”). I agree that LR is and shall be the exclusive owner of all right, title, and interest, including copyright, in and to the Material and the Works and further grant to LR an irrevocable, royalty-free, perpetual, and worldwide license to use information about my volunteer services in connection with the Material and the Works, without the need to notify me or seek further approval before doing so. 

8. LR Property. As a volunteer I may be entrusted with LR’s intellectual property including logos and trademarks and may be provided access to an e-mail address to use in support of my volunteer services. I agree that I will use LR’s intellectual property and any LR -provided e-mail address for official LR business only. I will not use LR’s intellectual property, business letterhead, or any LR -provided e-mail address for personal communications or personal gain. I have been given adequate time to review this Agreement and Release and I have fully read the terms of this Agreement and Release. I understand and acknowledge that by submitting this document I am waiving certain legal rights. By submitting this application, I accept and agree to the terms contained above.

9. I will not make health or financial decisions for LR. All decisions requiring finances will be approved by the rescue Director or Assistant Director and I will be held liable for any transactions not approved by the Directors. 

10. I understand that this can be a challenging environment and I will not promote negative actions or behaviors, in any way, to compromise the success and safety of this organization, its employees, customers, volunteers or animals.  L.O.L.A.’S Rescue holds a zero tolerance policy for mental, verbal, or physical abuse of property, animals or people. Anything perceived to be negatively affecting any part of this organization, will be diligently investigated and may lead to disciplinary action internally and externally, including termination and legal charges. The definition of Rescue is literally: “to save (someone) from a dangerous or distressing situation.”
